An invitation to the screening is sent to the Diya application on the 30th day after the person's birthday (if the person is 40 years old or older).
- After the person accepts the invitation, you need to order an Action.Card in the application or use your own, already issued one.
- Within 7 days, funds will be credited to the card - 2,000 hryvnias. They can only be used to undergo the 40+ Health Screening in a specified list of institutions.
- If a person does not have the Diya application, they will still be able to use the program. To do this, they need to apply through the ASN. The person orders a plastic card with a special account in certain banks (currently PrivatBank), contacts the ASN and applies for participation in the program. After about 7 days, the person receives an email notification that the funds have been credited and then continues the route in the same way as other program participants - chooses an institution and undergoes an examination.
- After receiving the funds, the person registers with a medical facility participating in the program. The list of medical facilities is published on a special landing page. screening.moz.gov.ua. Also, the list of institutions is necessarily published on the website of the National Health Service of Ukraine.
- Having determined a convenient institution on the landing page, a person will be able to immediately proceed to the appointment through the appropriate functionality of the medical information system (we recommend choosing this option).
- You can also make an appointment via a web form on the website of the selected medical facility, by phone through the reception desk, or during a personal visit.
- Healthcare facilities should designate specific time slots specifically for health screening. The recording system should mark such slots as available for health screening only.
- After making an appointment, a person should receive a confirmation (SMS, push notification or email), which clearly indicates: the date, time, and address of the health screening. And a certain time before the visit (24-48 hours) - a reminder with the ability to cancel or reschedule the appointment online, as well as recommendations for preparing for the health screening.
- At the designated time, a person must visit a medical facility for screening. Screening does not require special preparation. However, it is recommended to refrain from intense walking, smoking, drinking tea, coffee and other beverages containing caffeine for at least 30 minutes before screening.
- The visit lasts up to 60 - 90 minutes depending on the scope of medical services. The duration of the basic visit is up to 60 minutes (questionnaires, examination, collection of biomaterial, basic consultation).
- During the visit, the person pays for the service with the Action.Card.
- No later than 72 hours after the visit, the person should be sent the results of laboratory tests (lipid profile, HbA1c, and other indicators). Various channels can be used for this: the patient's account in the MIS, email, instant messengers.
- The indicators should be formatted in a «traffic light» format:
🟢 норма, також надається коротке пояснення, що все добре;
🟡 граничні значення – коротке пояснення, що потребує уваги;
🔴 патологічний стан – коротке пояснення та вказівка на подальші кроки (звернутись до сімейного лікаря, пройти дообстеження тощо).
Importantly! After passing the screening and paying for the service with Diya.Card, the funds will be automatically debited from your card. If you do not use the service, the funds will automatically return to the state budget, and you will be able to pass the screening again only next year.
