Our center has joined the national program "Health Screening 40+"!
We invite you to undergo a comprehensive examination for early detection of cardiovascular diseases, diabetes, and maintaining mental health.
The examination is funded by the state and is completely free for patients over the age of 40.
How to participate and get a card with funds from the state:
Through the Action app:
Open the application, find the "Health Screening 40+" service in the "Services" section and submit an application. Apply for a special digital "Diya.Kartka" at one of the partner banks. Within 7 days, it will receive 2,000 UAH from the state to pay for the checkup.
If you don't have a smartphone or Action:
Contact the nearest Administrative Services Center (ASC). The employees will help you apply for a special plastic card at the bank and enter the data into the system.
Important: the funds on the card are valid for 2 months from the date of enrollment and can only be spent on passing this screening.
