What is needed to make a declaration?
- Passport or other document confirming your legal stay in Ukraine.
- Identification code.
- Mobile phone to receive SMS code.
- For children: birth certificate and, if necessary, guardianship documents.
Re-submitting the declaration will not take much time! You can do this on the first floor of the outpatient clinic at 4-a Leonova Street. The process is quick, and you do not need to contact your previous doctor. The new declaration automatically replaces the old one.
