In order to implement the state veteran policy and strengthen social support for the families of the Defenders of Ukraine, amendments have been made to the Program for Supporting the Families of the Defenders of Ukraine, Families of War Veterans, and Family Members of Deceased (Deceased) Military Personnel in the Territory of the Chuguiv City Territorial Community for 2023-2025, approved by order of the Chuguiv City Military Administration dated 01.06.2023 No. 162, namely:
- The opportunity to compensate persons who received a disability as a result of injury, contusion, mutilation or illness while defending the Motherland, as registered in the territory of the Chuguivsk urban territorial community, for the cost of services received has been provided during independent sanatorium and resort treatment (receiving recreational services) on the territory of Ukraine.
Reimbursement (compensation) of the cost of independent sanatorium-resort treatment is carried out at the rate of: 50% of the cost - at the expense of the regional budget of the Kharkiv region and 50% - at the expense of the budget of the Chuguiv city territorial community.
Persons with disabilities as a result of the war from among the Defenders of Ukraine may apply to the reception office of the Social Protection Department of the Chuguiv City Council with a written application and provide the relevant documents to receive the above-mentioned compensation:
- document on payment for sanatorium treatment (original);
- copy of the voucher (original) or other document confirming the fact of receiving medical and health services, indicating the period of sanatorium and resort treatment (original);
- a copy of the applicant's passport of a citizen of Ukraine (all completed pages) (when providing a copy of the ID-passport together with an extract from the applicant's place of residence registration);
- copy of the certificate of assignment of the RNOKPP;
- a copy of the ID card of a person with a disability as a result of the war;
- certificate of opening a current account at an Authorized Bank.
