
Territorial Center for Social Services (Provision of Social Services) of the Chuguiv City Council provides social services to community residents who are in difficult life circumstances and need outside assistance.
In accordance with the list of social services approved by the order of the Ministry of Social Policy dated September 3, 2012 No. 537, the territorial center provides social services: home care, counseling, palliative care.
Social assistance department at home was created to provide social services at the place of residence/stay to single citizens who are unable to self-service due to partial loss of motor activity (have III, IV, V motor activity groups) and need outside assistance, providing social services at home according to a medical opinion, namely:
- senior citizens;
- persons with disabilities (who have reached the age of 18), except for persons with disabilities as a result of an accident at work or an occupational disease, who receive social assistance for permanent external care, household and special medical services in accordance with the Law of Ukraine "On Compulsory State Social Insurance against Accidents at Work and Occupational Diseases That Caused Loss of Ability to Work";
- persons with disabilities, including those with mental and behavioral disorders (except for persons who suffer from mental disorders and have committed socially dangerous acts and receive compulsory psychiatric care by court decision);
- sick people (from among single people of working age for the period before their disability group is established, but not more than four months).
The department does not provide social services to citizens who need 24-hour external care.
Specialists of the social assistance department at home:
- constantly identify, form an electronic database and determine individual needs in the provision of social services: elderly citizens, disabled people, sick people (from among persons of working age for the period before their disability group is established, but not more than four months), who are unable to self-service and require constant external assistance, recognized as such in accordance with the procedure approved by the Ministry of Health of Ukraine;
- ensure that an examination of the material and living conditions of elderly citizens, disabled people and citizens who find themselves in a difficult life situation is conducted, together with representatives of healthcare institutions, housing and communal services, and public organizations, with the identification of needs for the provision of various types of social services;
- promote the implementation of paid social services;
- ensure the implementation of innovative social services.
- In the home social assistance department, each recipient of social services is personally assigned a social worker, who becomes a professional representative, an intermediary in various community services, and a reliable assistant in solving life problems.
The main activities that make up the content of social home care services, namely:
- Assistance with self-care: assistance with washing, drying, bathing; dressing, undressing, putting on shoes; changing underwear; changing bed linen; bathing; washing hair; combing hair; shaving; cutting nails; using the toilet.
- Household management: purchasing and delivering food, medicines, industrial goods; preparing (assisting in preparing) food; cleaning (assisting in cleaning) the home; washing and ironing clothes, washing and pasting windows, carrying coal, firewood, lighting stoves, minor clothing repairs, clearing snow, delivering water from the water heater, helping with preserving vegetables and fruits.
- Assistance in organizing interaction with other specialists and services: doctor calls, utility services, transport services, visiting patients in healthcare facilities.
- Assistance in providing technical rehabilitation equipment, training in their use.
- Psychological support (conversations, communication, motivation for activity, psychological consultations).
- Providing information on public protection issues.
- Assistance in obtaining free legal aid.
- Assistance in preparing various types of documents, subsidies and making payments.
- Other social services.
Home care services are provided free of charge, on a paid basis, with differentiated fees depending on the income of the recipient of social services.
