Dear residents of the Chuhuyiv urban territorial community!
Full-scale military operations taking place on the territory of Ukraine have led to an increase in prices for basic goods and services, increased unemployment, a decrease in wages or a general loss of income, the negative impact of which has intensified on the level of well-being of citizens.
In order to determine the needs for social services and priorities for their further development, the Department of Social Protection of the Population of the Chuguiv City Council is conducting a questionnaire survey.
We invite community residents to participate in the survey and answer questions that will help make management decisions regarding the organization of social services.
Please read each question carefully and place a check mark next to your chosen answer.
This survey will help:
- identify and analyze the needs of the elderly, people with disabilities, individuals/families in difficult life circumstances, in social services;
- understand what services they need;
- analyze the quality of social services provided in the community;
- determine priorities for organizing the provision of social services;
- determine the state of information and awareness of the population about social services.
