To make a declaration, you need to have a passport or other document confirming legal residence in Ukraine, an identification code, a mobile phone to receive an SMS code, the phone number of a trusted person and, if necessary, documents on guardianship. For children, you need to have a birth certificate and documents on guardianship (if necessary).
Re-submitting a declaration is quick and without the need to contact the previous doctor. The new declaration automatically cancels the previous one.
