
Documenting individuals who cannot move independently due to a long-term health disorder is one of the important components of the implementation of the National Barrier-Free Space Strategy in Ukraine.
According to paragraph 36 of the Resolution of the Cabinet of Ministers of Ukraine No. 302 «On approval of the sample form, technical description and the Procedure for registration, issuance, exchange, forwarding, withdrawal, return to the state, invalidation and destruction of the passport of a citizen of Ukraine» dated March 25, 2015 and the Resolution of the Cabinet of Ministers of Ukraine No. 152 dated May 7, 2014 «On approval of the sample form, technical description and the Procedure for registration, issuance, exchange, forwarding, withdrawal, return to the state, invalidation and destruction of the passport of a citizen of Ukraine for traveling abroad», in accordance with paragraph 36 of the specified Procedure, in the event of accepting documents from a person who cannot move independently due to a long-term health disorder, upon the application of such a person or his legal representative, executed in writing, an employee of the territorial unit of the State Medical Service of Ukraine is sent to the place of residence of the person, treatment or to the location of the institution/facility where the person receives social services.
An employee of the territorial unit of the SMS during a personal visit to a person who cannot move independently due to a long-term health disorder checks and confirms the identity of the specified person and the person depicted in the photo, about which he draws up a report. The report is drawn up in the presence of the person/his legal representative, a representative of the institution/facility in which the person receives social services, and in the case of a person staying in a healthcare facility - also the doctor providing treatment. The report indicates information about the place, date and time of the visit, information about the employee of the territorial unit of the SMS and the persons present, the reasons for the visit, confirmation/non-confirmation of the health status and identity of the person. The report is signed by an employee of the territorial unit of the SMS, the person/his legal representative, a representative of the institution/facility in which the person receives social services, and in the case of a person staying in a healthcare facility - also the doctor providing treatment.
In the absence of persistent physical, mental, intellectual or sensory impairments, the person personally signs a signature on a separate sheet for further scanning using the Registry's tools.
An employee of the territorial unit of the State Migration Service, who, according to his official duties, is entrusted with the functions of issuing a passport, performs the actions provided for in paragraphs 24-29. 32 and 33 of these Procedures, and accepts for consideration the application form and the documents attached to it.
