Updating data in the declaration with a doctor: important steps for uninterrupted receipt of medical services

If your personal data or the data of your children in your declaration with your doctor does not correspond to the current information, be sure to contact your doctor with the relevant documents to make changes!

Why is it important to update data?

Timely updating of personal data in the declaration with the doctor allows you to avoid the risk of unexpected delays in receiving medical services. Up-to-date data in your declaration is the key to uninterrupted access to electronic referrals, treatment plans, prescriptions and other important medical documents!

What should you pay attention to?

1. Change of surname (due to marriage, divorce, at one's own request, etc.).

2. Obtaining a new identification code.

3. Replacing your passport (for example, with an ID card) or changing your phone number.

4. For children who have reached the age of 14 and have received a passport, it is necessary to replace the birth certificate data with the data from the new passport (ID card) in the declaration with the doctor.

For children under 18, it is also advisable to obtain an identification code (RNOKPP - registration number of the taxpayer's account card).

Why is this important:

1. Loss of access to the phone number specified in the declaration may result in the inability to receive electronic referral codes, treatment plans, prescriptions, and medical reports on temporary incapacity for work.

2. Changing your last name, passport, or identification code without updating your data will lead to errors when creating medical reports, such as an electronic sick leave, which may make it impossible to receive proper medical services.

How to update your data?

Call the reception at: 099-61-60-810, 057-46-40510, or contact your family doctor directly and make an appointment.

Prepare the necessary documents (passport, identification code, child's birth certificate, etc.).

On the specified date and time, come to the doctor's appointment to update your data.

Please note: If your phone number has changed and you are unable to receive SMS to the old number indicated in the declaration, you can contact the registry office or your family doctor with documents for submitting an application to the National Health Insurance Fund.

The application may be processed within several days.
